The earliest sighting of him is the ee_hallway map (with the 
Achievement):
After ee_hallway, Mörkö himself does not appear, but paintings, probably by the posessed Richard, can be seen in the bunker:
The first painting has a blurred-out Text, which says something like "The Underground God (which is probably "UGU") is dead. We need an Answer. Live"
From the Information in it, we can guess that Mörkö was probably around in the Bunker, but the player does not see him there.
The third appearance, and second time he is spawned, is the MulEx Copan Coffee Machine in stormdrain:
